Apply for a Health Declaration & Authorize the Driving School

You have registered with us for the driving licence B course — thank you! Before your practical exam can be scheduled, the CBR issues a Certificate of Fitness. You obtain this by completing a health declaration yourself on the CBR website. You must do this in person.

Authorize Us for Your Exam Request

To request your theory and practical exam, you must authorize us. This authorization protects you: a driving school cannot simply request an exam on its own, and you may only authorize one driving school at a time.

You authorize us using your DigiD. The steps are described on cbr.nl. Enter our registration number there so that we can schedule your exam. You can also search for our main location, Etten-Leur.

Make Sure You Have DigiD

You need DigiD for the health declaration. Don’t have it yet? Apply for it first via www.digid.nl. It takes a few working days before you receive it at home by post.

Log In to the CBR

Go to the CBR website and log in with your DigiD: mijn.cbr.nl.

Fill In the Questions

Answer the questions in the health declaration:

  • All questions answered “no”? Then the Certificate of Fitness follows immediately and we can reserve your practical exam.
  • One or more questions answered “yes”? Then the certificate cannot yet be issued, and, for example, a visit to a medical specialist or a driving test will follow.

Read the questions carefully and answer them truthfully. Not sure? Contact us — this may prevent unnecessary delay. Finally, you pay the fee for the declaration via iDEAL directly to the CBR.
